is there a definitive list of RoHS exemptions? i've read that military / defense applications are exempt and i've read that they are under review, but not exempt yet.
also, does one need to apply for an exemption or does one decide one is exempt and wait to see if the EU decides to investigate?

Yes, there is a list. I don't have it in english at the moment.
The time to apply is long gone. The exemptions just become less.
