A few months or so ago, some truck caught fire inside a tunnel on one of the freeways - it turned into quite a conflagration. I was kind of surprised to find that concrete isn't "fire-proof" - the trapped water boils, and pops out chunks of concrete - I believe this is called "spalling".
I also understand that this weakens the concrete considerably.

I checked Wikipedia. The fire damage included effects not properly considered spalling (generally a flaking off type of erosion and decomposition). Notably those included with steam explosions and chunks in excess of 1" largest dimension. Also note the MacArthur maze mess about 6 months earlier.
I have replaced the entrance cable on the side of a house, along with the meter base. I hack sawed the old cable from the drop, removed the damaged cable & meter base, ending at the fuse box. Then I worked the other direction, till I had reconnected the new cable to the drop. I never got as much as a tingle, because I used the proper tools & methods.
There are no "proper tool and methods" for working on live AC power connections.
Any time ANYONE does it they are doing it based on their balls, and there are no prescribed methods in place. All methods in any manual involve non-powered circuits.
That said, I always work on live gear as well. What one needs is an intimate knowledge of the fact that one cannot make one's self a pathway.

Remember that most concrete is reinforced, so there is a network of metal inside to channel the charge to ground. Even if the concrete is only marginally conductive, there is a LARGE cross sectional area, so you can get pretty good current flow...
