More likely the virus got him. A bunch of Chinese returning from Russia border region had a very high infection rate. You can expect the same in NK.

Difficult to say what "the population" believes about anything in a monolithic sense. Some are likely willing or unwilling participants of a delusional mass psychosis. Some know the score but find it beneficial to play along. And defections and escapes seem to happen regularly, even among older residents who say they had been fully aware of NK's bizarre pathology as compared to the rest of the world for some time.

Whoever assumes power (assuming he does die or become incapacitated) will do so through political maneuveri ng. The leaders seem to have no threat at all from the military. It's the other leaders they all worry about.
