Does anyone have info on how to make an adaptor to read these on an eprom reader?

Reading is simple - just connect the adddress & data lines up to the correspondin eprom ones and the eprom chip select to the Prom output enable.

Don't forget power and groumd. Duh.
I don't remember, but you have to choose the right CS, or not CS.
