This highly unusual aspect ratio would be a custom piece of glass, which will likely cost you in the region of $15k to get a one-off prototype built. Of course you have not specified whether this is monochrome, grayscale or color, whether it is to be backlit, or what the resolution requirement is.

Talk to the guys at Kent in Ohio; maybe a student there in their LCD labs will be interested in your project.

Yes, we can probably build 1 to 1000 prototypes for $10k, and around $5k for design changes.

The thickness requirements of 3mm would make it impossible for back- lighted LCD. However, OLED screen can be made with such requirement.

OLEDs are more expensive per unit, but NRE are roughly the same.
