One of my relative was significantly helped by ECT, to the extent that she requested it over the years. When ECT became unfashionable, she had to request it more forcibly.

It did seem to help her more than the drugs.

It's often portrayed as a scary or barbaric treatment in TV and film but in a modern medical setting it's not really like that at all.

I don't think it's known conclusively why it works, when it does. There seem to be a number of competing theories none of which seem immediately more plausible than another. I expect there's not a lot of money on offer to research the topic very thoroughly as vs. medications there's not a large commercial motivation, I'd guess there's only a couple of companies in the US that manufacture the equipment.

It's still used fairly often in the US for severe cases of various illnesses, in severe mania or psychosis it's likely life-saving sometimes.

The case I saw was the closest thing to a real-life miracle I ever saw, the person was on death's door, refusing to eat and they came right back. Like a "reboot", almost, I'm sure it's a poor metaphor for whatever is going on physiologically but that's what it seemed like. They lost some short-term memory of some of the period they were ill but long-term seemed mostly unaffected.

Additionally when it's used fairly often as a potentially life-saving emergency treatment for serious illness it's hard to form a control group.
