Usually because the prof or the app note said so. In a way we are all like that. When one of our dogs wouldn't stop vomiting we followed the vet's advice painstakingly. Until we found out he was wrong.
It's like popping a pain pill, just in case. Except that the opto isolator only adds cost while the pill can cause liver damage.
I do a lot of med gear that must be defibrillator proof and there you have no choice. Still, many times optos aren't the ticket plus there is always that scare that they are obsoleted. Toroid transformers can provide many kV but are hardly understood. Come to think of it, our profs didn't talk about those either.
