Sorry for the newbie question...
What is the right way to use a mica insulator on TO-220 parts? It seems to me that the supplied metal screw and nut would defeat the purpose of the mica, and conduct the TO-220 case to the heatsink.

You are missing a little plastic washer/grommet that keeps the metal screw insulated from the tab.
