I am looking for a CMOS camera with USB output or some other digital interface that I can directly interface to a FPGA.

Surely a webcam would suffice for USB? Otehrwise Kodak do a massive range of CMOS & CCD sensors that some complete with A/D converters - just give it clocks and it gives you data =D
I would go for a cheap webcam.
