Happens even in simple, discrete bipolar stuff like diodes. There it has to do with charge migation in the interface between Si and SiN passivation layer across the pn barrier. Failed part cones into analysis lab with terrible I_r. After baking out, everything is fine - for a while. Still fails after decapsulation. Then you etch off the SiN, and can't get it to fail anymore.

I don't remember the exact failure mode anymore, I'd need to ask.
