LTSpice IV doesn't have the UniversalOpAmp symbol, but it does have the UniversalOpAmp2 model.

The circuit I posted was modified from an older design which used this opamp, so possibly UniversalOpAmp got carried forward somehow. But it's in my LT Spice 4 library now.
