What second capacitor? One capacitor make sure that the average current is zero. The semiconductor diode makes the AC waveform have an offset equal to its peak amplitude. The tube sees a voltage that swings from zero to several thousand and back on each cycle.

It's an ordinary half-wave doubler, without the output cap. Presumably they don't mind the 100% modulation at 60 Hz. ;-)
