You may already be aware of this (hence prompting the question), but Wal*Mart (believe it or not) sells some with RCA jacks on the ends. Easy enough to use with an adapter or just whack off the end and re-wire if you'd like.

Does anyone sell a pair of isolation transformers in a little housing with 1/8" stereo plugs/jacks-- so that two audio devices with earth as common can be connected without ground loop issues?

It's kludge in my dictionary.
Radio shack sells them for about $16. I use one in my car to reduce ignition/alternator noise from the MP3 player in the trunk to the head unit in the dash.
It has phono connectors though not 3.5mm jack.

A search for "3.5mm ground loop isolator" turned up a number of hits. It's all in the terminology...

Never seen one. Given how expensive a decent audio transformer is, I don't expect you will see one either.
It's not 1/8" btw. It's 3.5mm.
