Looks like a common .025" square post header, with pins on 0.1" centers - made by AMP, Samtec, and many others.

Header for IDC. (IDC = Insulation Displacement Connector, a connector that is mounted on a flatcable by piercing through its insulation).
Also called: Molex header, dual row 0.1" header, AMP connector :-)
