This won't solve your problem, but it is a high pitched rear collision enunciator that gets attention.

I did this a long time ago on a huge 15-passenger van we had. You could back over a Volkswagen due to the high/small rear window in the huge back door.

So, I bought a cheap surveillance camera that output RS-170 video. I built it into a little box that mounted above the rear window, and ran DC and coax up to the front. The camera looked sideways into a 45 degree angled first surface mirror, so the image would come out looking like a rear-view mirror.

I mounted a "beach TV" up front and plugged the video into the video input plug.

Over time the thing deteriorated, but still worked. I never figured out whether the beach TV's LCD was degrading, or the camera was getting fried from sun reflections.
